ONLINE REGISTRATION WITH THE PRACTICE

To pre-register online follow these 3 easy steps:

NOTE: If all items below are not received, we cannot complete your registration.

1.  Click on the GMS1 link below to open the form. Once fully completed, completed click on the "SEND" button.

2.   Click on the NPQ link below to open the form. All patients MUST complete this New Patient Questionnaire in addition to the GMS1. (This form says for adults but is used for children as well). Once fully completed Click on the "SEND" button.

3.   Click on the email address below to send a copy of your photo ID and proof of address. Both are required for registration for Adults and MUST be provided. We will add a copy of your photo ID to your medical record.

>> Always ensure that we have your current telephone number, mobile number and email address, for when we need to contact you.

>> Inform us as soon as possible if you change your address. If your new address is outside our practice area, you will be asked to change your GP.
