REGISTRATION

Online registration is the preferred method at this time.

If you would like to register with us please ask our reception staff for details. They will explain our policy at the present time. They may then arrange for you to collect any necessary paperwork for completion prior to making an appointment for you to see the doctor for a health check.

You will need to complete a short medical questionnaire - this helps us to look after you until your medical record arrives. (You will need to bring a sample of urine with you when you attend.)

Registration forms and medical questionnaires can be found in "Registering Online" under the New Patients tab above.

PERSONAL DETAILS UPDATES

Please always ensure that we have your current telephone number, including your mobile number and email address, for when we need to contact you. Please inform us as soon as possible if you change your address. If your new address is outside our practice area, you will be asked to change your GP.

PLEASE NOTE: NHS numbers will appear on all documentation to inform you of your own number and to promote the use of the NHS number by patients.

GUIDE TO GP SERVICES

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
