Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports, driving requirements and adoption/fostering (HGV, PSV, AH1 & AH2 etc.)
  • Insurance claim forms
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates
  • 'To whom it may concern' letters
  • Forms for collection (see below)

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.


If you leave a form which needs attention from a doctor, it will only be ready for collection after five clear working days. This will be longer for doctors who work part-time. Payment can be made by cash or card for forms which incur fees (holiday cancellation, insurance, etc). A deposit of £10.00 may be required.
