Changing your contact details

To update details like your name, address, phone numbers or email address:

• fill out a change of contact details form
• phone or visit the surgery from 9am to 5pm

If you have moved to a new address, please complete the change of address form here. 

Please note if your new address is outside of our practice boundary, please read the section on registering as an out of area patient. 

• Check you are in our catchment area

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
